Enjoyed this on tap at Fat’s head.
A: Pours a dark brownish-black hue with a thin brown head that eventually winds up as a wispy lace.
S: Great toasted malt aroma with coffee and chocolate as dominate smells.
T: Smooth and crisp sweet chocolate flavors followed by coffee and cocoa powder tastes.
M: Medium body and minimal carbonation.
O: Great chocolate stout. Smooth and easily drinkable!

On tap at Fat Head's
A: Pours a deep dark brown almost black color with a thick tight brown foam head. THere is tons of lacing down the sides.
S: As it warms there is more milk chocolate flavors coming through. There is also some vanilla and some lightly roasted coffee.
T: Starts off with a nice tast of milk chocolate that is quickly evened out with a slight coffee taste. In the middle there is more sweetness, vanilla and milk chocolate. On the back end the nice sweet malt flavors are balanced by a nice amount of hops. Very robust.
M: Leaves a thick creamy taste on the palate. There is a slightly roasty coffee taste as well.
O: One of the better American Stouts that I have had in a long time. Easy to drink, a nice low ABV and a great robust taste. I would seek this out again!

Review from notes taken back in March.
On tap at the North Olmsted, Ohio location. Served in a nonic this brew appears black in color with some brown clarity around the edges and ruby clarity through the body when held to the light. Brown head forms across the top and recedes leaving a webbing of lace.
The smell of this brew is sweet chocolate notes with a fresh grainy aspect. Also included are notes of vanilla which highlights the chocolate and creates more sweetness to the nose. A little roasty with a touch of citrus hops thrown in to balance it out.
The taste is creamy chocolate grain with a mild roastiness mixed with a sweet vanilla inclusion. Dark lingering roast of the grain mixes with light citrus hop quality to balance out the would-be one sided sweetness of this brew.
This is quite an easy drinking brew that is easily approachable. It is medium bodied with a light feel of tingly carbonation. A light tingle of abv is noted in this brew. This offering is well balanced and something that would prove to be a good icebreaker for someone who is shy about the darker brews.

Had this at Fathead's in Pittsburgh, PA during the 'Burgh Brew Tour 2011. Poured from the tap to a snifter and ordered at the recommendation of the bartender.
Appearance: Pour is darkest brown/near black; starts with a very thin skim of a dark tan head; body is dark, nearly opaque and I can only see a few lines of carbonation bubbling up near the sides of the glass; a few swaths of lacing end up getting left behind.
Smell: Bit of a sweet and syrupy chocolate smell with some bitter sweet baker's chocolate included; however, there is something going on here that doesn't set quite right, as I'm picking up a strong scent of butter... uh oh... could it be diacetyl? Could it be a dirty tap line?
Taste: Uh-oh, a lot of buttery diacetyl definitely makes its presence known; sweet chocolate follows with a touch of earthy roastiness of a chocolate infused coffee drink; could be better, but it could be worse; slight cherry flavor in there as well. I feel that a better offering from a well cleaned line or from a bottle would give a better representation to what they were going for.
Mouthfeel: Body is of medium weight, and the carbonation is relatively strong for a stout; a thin, semi-moist coating leaves a little to be desired.
Overall: It could be a couple of things here, perhaps dirty tap lines, or perhaps a beer that was brewed without some of the care that others get, but there was definitely buttery aspects of this beer that hurt its score However, I would be more than happy try it again, based upon the atmosphere of the restaurant alone

On tap @ Fat Heads Cleveland. Served in a pint glass for around $5.
This beer looks OK in the pint glass. It’s more of a dark brown than a black, with some hints of caramel and even red swirling around. The off-white head has left the building; spotty lace is all that remains. More head retention and better color (darker and more dense black) would make this score better. It doesn’t look bad, but it’s not great either.
The nose is full of cocoa powder notes from the roast. There’s also a bit of sweet malt, lactose and some caramel. I’m not getting the dark Belgian chocolate or the vanilla bean that the commercial description mentions, and the smell is subdued compared to great sweet stouts and milk stouts. However, it’s pleasant enough and has appropriate balance of scents. Kick it up, make it more intricate and we’ll be talking 4.5 or higher.
The taste is also good but a bit short on unique and explosive flavor. The experience starts off with some bitterness from roasted malt and hops. Then, the dark chocolate and powdered cocoa flavors hit, followed by a touch of sweet caramel and maybe, if I squint my eyes and taste buds, some vanilla. There’s a slight drop-out of flavor, then some lingering roast and sweetness. Not enough personality in the taste here.
The mouthfeel is terribly thin. I’m surprised this actually has lactose sugar in it. The carbonation is OK – somewhere directly between prickly and creamy – but the body just doesn’t cut it here in the big leagues. I think this beer could be something special. From the stats and ingredients, I expected big flavor with a dense mouthfeel. While it’s not a bad beer by any means, this is way too textbook for me. Bring some more flavor, please!

Voodoo Monkey Chocolate Stout
Pours a very dark black, light head forms and fades away. Aroma is comprised primarily of chocolate and vanilla. In the taste, a nice blend of vanilla, roasted malts, and finally cocoa on the end. Certainly a stout with alot of roast and coffee flavor. I was expecting more from the imported ingredients but they were very mellow and only imparted slight flavors. Overall it was still a nice session beer at Fatheads and was a dessert in itself.

From notes at the brewery:
A luxurious chocolate stout brewed with 70% cacao dark chocolate from Belgium, whole vanilla beans, and milk sugar for a rich creamy stout that is a perfect match with the slow smoked pulled pork. There are clearly supernatural forces at work here we are not sure what they are so it is assumed to be Voodoo Magic.
Review from notes. Enjoyed by me on tap 12 23 09 into a clean, clear pint glass with my brother in law Steven and my wife at the brewpub at that time.
A Nice medium brown head over what looks at first to be a dark brown body but holding it up to direct light reveals some dark orange hues coming through. 4.0
S Dark roast malt, chocolate. Yummy smelling. 4.0
T Dark roast malt and chocolate carries right over into the taste in a great balance of sweetness and roasted goodness. I am very impressed by the taste of this beer. 4.5
M Creamy feel, exactly what I like in the style. 4.5
D Very good. I found the bottom of my pint reasonably quickly. 4.0
Notes: One of my favorite Fat Head's offering and I have had many of them!
