Hoppin Frog - it's a must if you're in Northeast Ohio
    Bells
Alesmith - BA Speedway and variants - nothing left to say
    Founders
The Bruery - always a bunch of BA beers on tap

    Honorable Mention:
    Great Lakes
    Firestone
    Iron Fist
    Port/Lost Abbey

Never been a big fan of the Stone brewery. Only a few of their standard beers to do in tasters. Nice taplist otherwise. Beautiful outdoor dining though.

In order of my scores for reviews done (plus some wiggling on a couple of them).
    1. Russian River
    2. Jackie O's
    3. Short's
    4. Founders
    5. Bell's
My wife would like to add Hair of the Dog into that top 5 too, so there's a runner-up name for you.

Edit: after seeing the following post, I'll add that my list is self explanatory for short comments.

Makes sense about Peg's. I still consider them connected. It's Doug's mom's place, and they still consider it a brewpub. Yeah, Coppertail is not in a location that most folks would find themselves often unless you work in that commercial district. Great place once you walk in.

Russian River - Those beers on tap, best yet
Bear Republic - I was able to stir some mash, bucket list item
Founders - Sweet outdoor area, good vibes for a Midwest place
Stone - The place is as San Diego brewery as it gets
Three Floyds - All brews on tap are affordable enough to try and drink all of them in one sitting

Town Hall, Minneapolis (not necessarily for everyday visits, but I went during the week long Barrel Aged Festival, which is fantastic enough to put them at the top of my list)
Cascade Barrel House, Portland. Not only great sours, but pretty fine food as well.
Russian River ... no elaboration needed
    La Cumbre, Albuquerque
Elevation, Poncha Springs CO (obscure off the beaten path brewery doing fantastic beers)

Footnote: as a Seattle guy, I notice I don't have any Seattle breweries on the list. Holy Mountain, a new brewery, is very close to the top five above. And I can heartily recommend Reuben's, Fremont, Sound, or Black Raven in our area as well. But as it happens, my most memorable brewery visits are from elsewhere.

I need to try harder. Both Lagunitas and Firestone Walker are within fairly easy driving distance. I'll stop by Lagunitas on the way to wine country in 2 weeks.

    In no particular order...
Ithaca- the new digs are great. My son could play bocce and shuffleboard and drink root beer all day there. Me too minus "root."

Ommegang: go during the fall. It is great to see the brewery from outside just before dusk with the hills blazing with fall foliage.

Great Lakes: one of my favorite breweries, period. I've been there in all seasons and it's all good

Troegs: great regular beers, great one offs. I hate Mad Elf, but loved Naked Elf (Mad Elf before they add the honey)

McNeill's (Brattleboro, VT): this one's a sentimental pick. A buddy of mine lived in East Dover for a while and we would always go to McNeills. Solid beers (loved their Alt and ESB) neighborhood vibe.

I've only been to 4 Breweries in my life:

1.Founders. I live 6 minutes away and frequent often for fresh and canned brews.
2.Leinkugels. Old brewery with a historic tour and friendly inviting feeling at the Lodge.
3.Millers. Milwaukee, WI. Great history of American brewing here.
    4.Budweiser. St Louis MO.
