A perfect 5 is a beer that tastes so awesome that at that moment you cannot imagine anything that could possibly change to make it better. I'm talking total mouth-gasm!

That being said, I've rated a few perfect 5's but later when I tried it again lowered my rating by a few tenths.

And there's the "problem" with different raters. Though not really a "problem" everyone treats and thinks about the 5 point scale a little differently. Personally, I've never given a 5, out of the 400+ I've rated I think the highest i've given is a 4.7something.

Some beers used to be 5's, but not any longer.

I'll say it: fresh Heady Topper. To me that beer cannot be improved upon and exemplifies the style.

I try to reserve 5's for beers I don't think can be improved within the style they portray, but I also don't take it as seriously as some.

I think Rochefort 10 was the closest I ever came to a 5 (not on this site)

Rating a beer a 5 is like getting married: Sure you're with someone you think is great but is there someone out there better? Maybe.

Some raters equate a 5 rating with perfection, and that it can not be obtained. I give out 5's like Daym drops.

I try to make a point of actually making 5's attainable.. definitely not often, but when I see fit. Palates change, even your absolute favorite beer not probably won't be eventually in the future. I like to rate in the moment, not "if I drank this again in 50 years, would it still be amazing," and also within the style. Even if it's not the best imperial stout or DIPA, say, if it's everything I could ever want out of a brown ale, or English bitter, I give it what it deserves.

I gave FFF Permanent Funeral a 5 because I don't think there is anything that could possibly be done to make that beer better. I gave FW PNC Buckwheat Stout at 4.75 but it is oh so close to being a 5 in my mind. And while I didn't rate it a 5, Cellarmaker Imperial Coffee and Cigarettes may not be improvable. In my mind, a perfect beer is one that cannot be improved so there are a few that may make that cut.
