FFF's varied art styles makes every bottle seem like a special treat and certainly grabs your attention, even if it takes a while to read the font and figure out what the hell it is (YUMYUM).
Unibroue bottles have a cool and recognizable art style, and if you go on their website you'll learn about some Canadian lore to go along with it.
I think Surly is trying to be like these:
but don't take it far enough. C'mon, really offend me!!!

I'm also putting out another local-ish brewer, Apocalpyse Ale Works.... I dig the skeletons/undead/hellfire thing.

I don't see a lot of these as themes, more as branding. For a brewery that has a true theme, one which encompasses everything from the taproom (decor, music, ambiance), the beer itself (names, label art), the owner/brewer/employees (dress, mentality, personality), and everything in between, I would say TRVE in Denver absolutely takes the cake.

Phantom Carriage is a brewery with a horror movie theme, complete with decorations and a horror movie screening room.

Ballast point brewing pirates and fish. Just gives me California nostalgia every time (home state)

Stones gargoyles and thesaurus researched vocabulary .. Too many now though, I just drink the beer at this point..

Lagunitas for the insinuation "of we don't take labels seriously but our beer is world class "

Modern Times with their book theme and great branding and Bottle Logic with their sort of science theme and also great branding.

St. Francis Brewing in St. Francis, WI names their beers after the 7 deadly sins (Lust, Greed, etc...) Thought that was pretty clever given the name of the brewery. The beers are pretty meh though.

Only one that I can think of that I found creative would be Black Raven in Redmond, WA.

Kane Brewing(my favorite brewery) in Ocean NJ has a bit of a surfer theme as Michael Kane is a surfer. Their core beers (Head High, Overhead, Single Fin, Cloud Cover and Drift Line) all get their names from the owner's favorite pastime.
Again, themes are only as good as the beers themselves and theirs are very very good!!!!

That is a cool theme. Hopefully they don't get sued by Apocalypse Brew Works in Lousiville. And hopefully they are a hell of a lot better.

Base camp brewing in Portland is a hoot. Haven't been there personally but I've seen many pics and love there beer. Looks like such a good time there.

Burial Beer Co. for me. Every beer name and its respective tap handle is either a tool for digging or farming or something to do with death or rebirth. Just a real cool concept overall:

Burial, for some, is a necessary step to reach the afterlife. At Burial Beer Co., we see it as a celebration: of life, of the cyclical nature of harvest and of the brewing process. We find glory in the things that once were

If I ever find the start up money I would want to name my beers after different salmonidae species and subspecies. S. Trutta brown, O. Clarki ABA, T. thymallus saison, O. chrysogaster chelada, and so on. I know very well that it wouldn't catch on, but I can dream.
