Can't Submit a Place

I have been trying to add a place since yesterday evening and keep getting a "500 Internal Server Error" message each time I try to submit it. The URL of the page is: http://www.beeradvocate.com/beer/add_est/.

I was able to submit some other places in Ireland today but ran into a problem with one brewery and got an red warning message saying "establishment may already exist in our database. search BeerFly for: N17" before I could submit it. I didn't see any places called "N17" in BeerFly, just some postal codes in London. N17 seems to be an unusual name for a brewery but it's located near the N17 motorway. I tried submitting the name as "N17 Brewery" but got the same warning message and it wouldn't let me submit it. Here are the entry fields:

Were you able to adjust the code? I tried submitting N17 again, but still get the error message that the establishment may exist in the database.

 

I put n/a into the fields and didn't leave them blank.
