Breckenridge '72 Imperial or Samuel Smith's Organic Chocolate Stout are my two favorites.

    ALL. OF. THEM.

    But seriously:
    ST Choklat/Choklat Oranj
    ST Mokah
    Sam Smith Organic Chocolate Stout
    GD Chocolate Oak Aged Yeti
    Brooklyn BCS
    ET Soft Dookie
    Rogue Chocolate Stout
    YDCS

And then there are others which aren't entirely designed to be chocolate forward (given the presence of other adjuncts) but still offer up huge chocolate aromas and flavors:

    Prairie Bomb!
    Prairie/ET Bible Belt
    Stone Xocoveza

Aged FBS for sure. Its pretty coffee forward when it first comes out, but let it sit for a few months and you will be pleasantly suprised.

Prairie BOMB! is pure chocolate to me. So damn good.

Sam Smiths Chocolate Stout. Readily available and very reasonable priced! Delicious and with its low ABV, its guzzle-able!

Feel the same way. Out here that's the only way to drink it. Like a big old thick frothy chocolate shake. Damn good!
