-NOLA in New Orleans has a fantastic tap room, and they've just made their 3rd and very large expansion earlier this year. Multi-floor, a game room that will be turned into a music venue soon as well. Have great BBQ as well.
-Avery, lucky to have visited there. phenomenal facility and taproom.
-Foley Brothers - small, but family run and on a nice piece of land.
-Hill Farmstead - haven't even been to the new taproom, and I imagine it is incredible.
    -Lost Nation
    -Maine Beer Co.

Logboat because of their Shipyard. Tons of space for lawn games, food trucks, picnics, etc.

I think it's a bit difficult to say here, considering what one is looking for in a tap room (as well as what one considers a taproom in the first place) will vary wildly from person to person. (atmosphere/decor, beer variety, beer quality, size/crowding, food available, tours, events/releases etc.)

Saying that, I'm really not sure what I'd call the best, but one that truly sticks out to me is Lost Nation in VT. Awesomely apropos setting in a barn you'd never notice was a brewery unless you were looking for it, quality beers all around on my tasting flight, and the pork belly sandwich at their open BBQ pit could very well be the best sandwich I've ever had.

Favorite brewery taprooms would be Founders and Avery..
Favorite taprooms that aren't at breweries would be Max's in Baltimore and Falling Rock in Denver.

As far as bars/pubs are concerned, Blackback in Waterbury is my favorite.

For breweries, my favorites would be Cantillon and I'd say 14th Star because it's really cool in there.

Brewery Vivant was pretty dang cool, as was Brewery Terra Firma.
I have an affinity for old warehouses, but that's because I was a raver who remembers the days of old dirty warehouses, and rave boogers. So I give a nod to Revolution, and Temperance. I just cannot stand it when Rev has office/cube farm parties, and the bar is clogged with a bunch of bros wearing ties with no clue what to drink other than whatever happens to have the highest abv.
Tugboat in Portland, Oregon remains one of my all time favorites, but that's because I spent huge portions of my early 20's lingering in coffee shops smoking loosies and drinking until I vibrate. And that place oozes that vibe.
