Ouch, @Soneast, I feel your pain. My latest blunder isn't quite to that level, but it's also very recent. I brewed my first black IPA yesterday, which I had prepared using @VikeMan's excellent BrewCipher spreadsheet. As I typically do, I took a similar recipe, copied the sheet and edited the ingredients. But what I didn't notice was that I forgot to erase one line from the previous recipe - the line that called for adding a pound of sugar. Looks like my BIPA is going to be an Imperial BIPA! It's not a total disaster, but I don't know if I hopped quite enough for a 9% beer, I'm probably going to end up with around 70 IBUs. Good thing I like strong beer!

Probably the worst thing that has happened is the output hose from my 50 ft. wort chiller came dislodged and sprayed boiling hot water into my friends shoes. I feel so bad everytime I see the scar on his foot.

On a recent IPA my bottling wand was getting clogged with hop sediment. The result was beer flying everywhere after sending the hose rocketing off the racking cane. Not once, not twice, three times.

I now move all my beers to a bottling bucket before bottling.

Haven't really had any epic fails, but deciding which mitigation procedure (if any) to employ when things go wrong can be a crap shoot.
