Love blind tasting. This sounds like a tough one, and the results are interesting. I agree that it boils down to if you have either had the beer very recently, or often to be right on all counts.

Personally, if I were involved, I may have allowed a 2% range to ABV guesses. Some friends and I guess often when we try new beers what the ABV is on the bottle, and are sometimes surprisingly dead on, or way off, but often were pretty close (within 2%)
Vintage, I'd go with multiple choice on that one. Is it Fresh, a year, or 2+. Guessing the brewery that made it, a bit easier on rarer offerings, especially those one may have had a few of before, but guessing one brewery's IPA, or Dry Irish Stout over another, really difficult imo. Style would be fun, and easier to guess, but still difficult in a lot of cases.

Best thing about Blind tastings, is removing assumptions, perceptions, hype from the equation. The results are more times then not, in my experience, surprising.

I would think Brewery and Brew Name would be very, very difficult to nail. and ABV, what if you say 5.4% and it's 5.5? Yeah that would be almost impossible too.
